| Abstract | 目的 调查深圳市CKD患者并发结核病的发生和预后情况,探讨CKD患者并发结核病的基本特点.方法 选取2013年1月至2017年12月深圳市四所医院肾内科与血液净化中心接受CKD治疗和透析治疗3个月以上的患者,分为终未期肾病(ESRD)和非终未期肾病(no-ESRD)两组,回顾性分析两组患者的结核病发病率、临床特征及预后.结果 ESRD组和no-ESRD组分别有1169例和3062例患者入组.ESRD组有57例(4.9％)确诊为活动性肺结核,其中肺结核36例(63.2％)、肺外结核14例(24.6％)、并发两种疾病者7例(12.3％),死亡率为8.8％(5/57).在3062例(2.3％)no-ESRD患者中,有69例诊断为活动性肺结核,其中47例(68.1％)为肺结核、16例(23.2％)为肺外结核、6例(8.7％)为并发两种疾病者,死亡率为2.9％(2/69).ESRD组与no-ESRD组比较,发病率和病因分布的差异均有统计学意义(x2值分别为19.502、10.076,P值分别为0.000、0.018)；而两组在免疫学检测、结核类型、临床预后方面的差异均无统计学差异(x2值分别为0.222、0.530、3.971,P值分别为0.974、0.767、0.137).结论 CKD患者并发结核病总体发病率高,ESRD人群中结核病高发.迫切需要对CKD患者进行结核病筛查,以早期诊断并治疗. |
