Abstract
目的:在三维有限元模型中,比较不同包裹面积及不同步距设计的无托槽隐形矫治器,对伴有不同程度牙槽骨吸收的右上中切牙压低效能的影响.方法:建立含3种包裹面积(100%、75%、50%)、3种步距设计(0.1 mm,0.15 mm,0.2 mm)的无托槽隐形矫治器三维有限元模型,模拟3种牙槽骨水平吸收状态下(牙槽骨正常水平、水平吸收1/3、水平吸收1/2),压低右上中切牙时,其位移及牙周膜应力的变化.结果:除牙槽骨水平吸收1/2、包裹面积50%、步距0.1 mm组外,各组右上中切牙主要表现为:在垂直向上近中切角伸长,其余部分压低,近中切角伸长量小于根尖压低量;在唇舌向上牙冠及牙根舌向移动,近中切角及切缘位移量大于根尖.另外,随着矫治器步距的增大,右上中切牙位移量也随之增大.各组右上中切牙牙周膜总体应力分布较均匀,腭侧面应力值大于唇侧面,应力值随矫治器步距增加而增加.矫治器步距设置为0.1 mm时,除包裹面积为50%的矫治器组产生的矫治力小于150 g/cm2,其余组矫治力均介于150~260 g/cm2之间;矫治器步距设置为0.15 mm和0.2 mm时,各组矫治力均大于260 g/cm2.结论:当无托槽隐形矫治器厚度固定为0.75 mm,不同牙槽骨吸收状态下,矫治器包裹面积大于50%,步距设计为0.1 mm时,最有利于在安全矫治力范围内(150~260 g/cm2)实现压低右上中切牙的高效矫治.
